As per approval of the UECFI Board of Directors General last September 13, 2009 regular board meeting held at the Old General Center, Quezon City, the KKEP National Board of Directors will distribute the UECFI Official Directory Form to all affiliated Local Centers of the Union to be accomplished by the Katandaan Local Officer(s). As one of the KKEP National Board's projects, this aims to update the UECFI Directory of Affiliated Local Centers which was published in 1990s. Guidelines in filling up the forms: 1) One (1) form for each Registered UECFI Local Center; 2) ALL REQUIRED fields as denoted by an asterisk (*) in the form MUST be filled-up. Incomplete forms will be returned back to the respective Local Center and will not be included in the update of the directory; 3) Entries in the form should be clearly stated or written; 4) Completed forms MUST be signed by the respective UECFI Local President of the Local Center, UECFI Regional Secretary and UECFI Director General; 5) UECFI Local Secretary must put his/her initial prior the signature of the repsective UECFI Local President; 6) Submit the completed forms to your respective KKEP Directors for consolidation; 7) DO NOT fill up the Control Number at the lower right portion of the form as this will be completed by the KKEP National. All completed forms and the official listing of local centers must be submitted to the KKEP National Board of Directors not later than October 31, 2009. To download the form, click here.
